Dhiren N. Ajudia, MD has been approved by USCIS/INS to perform medical examinations for U.S. immigration dept, commonly known as I-693 forms. We have been doing these type of examinations since 1997. We are very skilled in following INS requirements and then give you a properly completed examination in sealed envelope for INS and a copy of complete examination for your own records.

If Your Age is...	You Will Need...	Comments
Adults and Kids 15 Yr & Over	1. Fill out history forms provided by us. (or Download here) 2. An examination by our INS-approved doctors. 3. Blood tests for Syphilis (RPR) by us.(HIV test not required from 1/4/2010). 4. Tuberculosis SKIN test by us unless you have proof (documentation) of having Positive (Abnomal) skin test in the past. 5. Bring in the proof (original or photocopy) for the required immunizations if you have it. (See minimum age-specific immunization requirements) 6. You will need to return between 48 -72 hours to get your TB skin test read by us. Chest x-ray will be required if your TB skin test reaction comes positive (abnormal) or If you had a proof for positive test in past. Under 15 Yr	1. Fill out history forms provided by us. 2. An examination by our INS-approved doctors 3. Tuberculosis SKIN test is NOT needed unless some one in the family is having a tuberculosis currently or recently. 4. Bring in the proof (original or photocopy) of all your immunization records. Bring in all of your immunization records for our doctor to review. We can provide/recommend any missing doses for us to complete your medical exam & paperwork. Additional doses/series can be completed later with your own doctor.	If you have not started or completed any series, you may need at least one dose to start/finish each series for us to complete your INS paperwork. You can finish series later on.
